The British Pain Society (BPS) have a booklet introducing how to understand and manage Chronic

Pain (note this resource is due to be renewed - but is at present still offered by the BPS)

The BPS have also produced a patient care pathway, or map, focused on the initial assessment of chronic pain. Note that this is primarily intended for use by medical professionals.
